Get Immunized

Our Nursing Division works to create a healthy community through the control of vaccine-preventable diseases. Residents can come in 7:30 a.m. - 5:00 p. m. on Monday, Tuesday, Wednesday and Friday for vaccinations.  The first and third Tuesdays of every month the hours are extended to 7:00 p.m.

The Ohio Department of Health and the federal Vaccines for Children Program supply the Health District with vaccine for children 0-18 years of age.  These state supplied vaccines are offered for a minimal administration fee.  No one is denied state supplied vaccine due to inability to pay.  Additional recommended vaccines are purchased by CCCHD for children and adults.  The fees for these vaccines vary due to changing costs from the manufacturers.  Please call us for more details regarding vaccination prices: 937-390-5600.  Be sure to bring your immunization records along with Medicaid, CareSource, Molina, Amerigroup or Anthem insurance card with you for your service.

Do you need to know if you or your child is due for a vaccination? We have printable schedules for ages 0-6, 7-18 and adults 19 and older.  The 2012-2013 Child Care, Head Start, Pre-School and School Attendance Immunization Requirements can be found by looking at the Immunization Summary for Child Care, Head Start, Pre-School and School Attendance documentation.  Satellite clinics are held throughout the county on a monthly basis. Need to Find Your Immunization Record?  When a vaccine is given to you or your child, be sure to ask the provider of the vaccine to give you a copy of that vaccination record.  Future vaccines will be decided based on the information on that shot record or vaccination record.  Additionally, re-entering certain career fields may require you to produce a vaccination record and finding that vaccination history can be difficult if you do not have a copy of your record.  If you think you may have had vaccinations at the Health District, please call (937) 390-5600 to receive a copy of what vaccinations we have documented for you.

*Immunizations are given according to the schedule recommended by the Centers for Disease Control, Advisory Committee on Immunization Practices, American Academy of Pediatrics, and the Ohio Department of Health and per standing order of the Medical Director for the Health District. Adult clients and the responsible adult of minor children will fill out a questionnaire at each visit regarding health status and any reactions to previous vaccines administered. They have the opportunity to ask questions and have the information on the Vaccine Information Sheets (VIS) clarified if necessary. Clients under the age of eighteen years must be accompanied by a responsible adult to receive services.
