Vision:  Making the Healthy Choice the Easy Choice.

Mission:  Creating Healthy Communities (CHC) is committed to preventing and reducing chronic disease statewide. Through cross-sector collaboration, we are activating communities to improve access to and affordability of healthy food, increase opportunities for physical activity, and assure tobacco-free living where Ohioans live, work and play. By implementing sustainable evidence-based strategies, CHC is creating a culture of health.

Join Us!

The Creating Healthy Communities Coalition meets monthly in a variety of locations throughout Clark County. At the meetings we discuss project and programming updates and members are able to network and connect with those in the community pursuing similar goals. Please feel free to join us, and be a voice in YOUR community for healtheir living! Quarterly full coalition meetings are scheduled for the following dates in 2018: January 23, April 3, September 18 and November 13.  All quarterly meetings will be held at the Clark County Combined Health District offices located at 529 E. Home Road, Springfield, OH from 3-5pm.
