As Clark County’s central source for genealogy records, we can provide:
- Birth information for anyone born in the State of Ohio back to 1908.
- Death information for anyone born in Springfield or Clark County, Ohio back to 1908.
Do you need to lookd further back? Birth and death records from 1867 - 1908 are available from the Springfield-Clark County Heritage Center and the Clark County Probate Court or those who were born or died in Clark County.
Getting documentation from our office is quick and simple. Certified birth and death certificates cost $22.00 each. You may obtain birth or death document from Vital Statistics through any of the following options:
- Walk-in: Stop by our main office at 529 East Home Road or call (937) 390-5600 to place your request.
- Fax: Send your request in to (937) 342-5503
- Website: Don’t even bother leaving our site! Simply hop over to our request form page (link to page) and let us know what you’re looking for.
- Online: Visit www.vitalchek.com to order your certificate online. Additional shipping/processing fees may apply.*
- Mail order: Simply print out and complete a form below and send it in to our office with either a cashier’s check or money order payable to CCCHD and a self-addressed stamped business-sized envelope. Certificates will be mailed out the following business day after we receive your request. Cashier’s checks or money orders should be in the amount of $22 per certificate you are requesting along with an additional $1 for shipping.
