The Clark County Combined Health District houses birth and death records from 1908 to present for those who were born or who died in Springfield or Clark County Ohio.All birth and death certificates are certified. We do not issue uncertified certificates.
The cost for each birth or death certificate is $17.00.
Vital Statistics offers the following services:
- Walk-In: Certified copies of birth and death certificates may be obtained at the Health District's Main Office. Cost of each certified copy is $17.00.
- Order On-Line: at the VitalChek Website www.vitalchek.com. The cost is $17.00 plus any shipping and processing fees. For information on the shipping and processing fees, please go to the Vital Chek Website.
- Mail-In: Print, complete and mail the appropriate form:
Birth Certificate Request Form or Death Certificate Request Form to the
Health District's Main Office, along with:

- A cashier's check or money order payable to CCCHD, in the amount of $18.00.
- Please include a stamped, self-addressed, business-sized envelope.
- Once we receive your request, the certified birth or death certificate will be mailed the next business day.

Birth and death records from 1867 - 1908 are available from the Clark County Probate Court
